What is the aim of the Community Health Centers Grants?

To improve the health of the Nation’s underserved communities and vulnerable populations by assuring access to comprehensive, culturally competent, quality primary health care services. Individual health center grant mechanisms include: (1) Community Health Centers; (2) Migrant Health Centers; (3) Health Care for the Homeless; and (4) Public Housing Primary Care Program.

Who is Eligible to Apply for the Community Health Centers Grants?

Eligible applicants are public and non-profit private entities, including tribal, faith-based and community-based organizations that have the capacity to effectively administer the grant.

How to Apply for the Community Health Centers Grants?

HRSA requires all applicants to apply electronically through Grants.gov.

All qualified applications will be forwarded to an objective review committee. Based on the advice of the objective review committee, the HRSA program official with delegated authority is responsible for final selection and funding decisions.

How much is the Community Health Centers Grants?

$200,000 to $11,000,000.
