What is Blue & You Foundation for a Healthier Arkansas Grants?

Established in 2001, the Blue & You Foundation was established and wholly founded by the Arkansas Blue Cross and is part of Blue Shield’s legacy of care and commitment to the welfare of Arkansans. The Foundation intends to provide caring solutions to Arkansans’ health-care needs and promotes the commitment of the Company to the public good and corporate citizenship.

The Blue & You Foundation seeks the power of partnership in order to achieve their goal of improved health for all the Arkansans. The grants are strategically focused on funding community-based solutions and hopes to produce a positive impact by nurturing community health leadership, fostering collaborations and innovations and leveraging human, financial and community resources. The primary mission of Blue & You Foundation is working together with others in creating a better state of health for Arkansans.

Who are eligible for Blue & You Foundation for a Healthier Arkansas Grants?

To be eligible for the Blue & You Foundation Grants the organizations must meet the following criteria:

  1. An Arkansas-based organization or a government agency. If the organization that is applying is located outside the jurisdiction of Arkansas, the project that will be funded must benefit Arkansans.
  2. The non-government organization must have a current tax exempt designation under the 501(c)(3) Internal Revenue Code.
  3. Must not be a private foundation under the IRC 509(a).
  4. The primary grantee must not have a contractual relationship with Blue Shield and Arkansans Blue Cross, its affiliates or subsidiaries.
  5. Grants are not awarded to: fundraising celebrations or events; individuals; lobbying or political organizations; fraternal, social or athletic organizations; organizations who don’t directly serve Arkansas; religious organizations with religious purposes; endowment or capital of organizations; proposals principally for large equipment and/or facilities; or in attendance of conferences.
  6. The Foundation does not fund “indirect costs”. Please omit these costs from your budget and request.
  7. The Foundation does not fund tobacco-related proposals due to the funding availability in Arkansas from its Tobacco Settlement.
  8. Those organizations that have already received funding from the Foundation for a specific program for two years in total are no longer eligible to reapply for more funding in the same program.

How to apply for Blue & You Foundation for a Healthier Arkansas Grants?

To apply for the Blue & You Foundation grant the application must be submitted with the following contents:

  1. Cover Pages
    • Use the specific 516 KB PDF Form as your cover page. This is downloaded from the Web site link. Fill out the form online. Then print, sign and include the materials and descriptions specified below.
    • The Cover Pages is a brief and consistent 2 page format providing information about the organization and a brief summary of the grant proposal. The presentation of the project summary should be a clear and concise statement of the proposed project. The cover pages must be the “Executive Summary” containing the most compelling points stating why the project must be funded.
  2. Project Description (a 3-5 page narration)
    • Goals and objectives.
    • Principal methods and activities on how these goals will be implemented.
    • Timeline for the year on how you will achieve success.
    • Geographic area that will be served
    • Target population (age, gender, etc) that will be served
    • Assumptions where the project is based
    • Barriers to your success
    • Human and financial resources that will applied to the project including the expected support from other organizations and the community
    • Likelihood of the project’s continuance after the period of the grant including other probable funding sources
  3. Project Evaluation (one page)
    • How you will measure your success
    • The tools and data you will use to verify success
    • Evaluation timeline
    • How you will identify and correct your project problems
  4. Project Budget
    • List in detail all revenue sources and project expenses. (Grants will not be awarded to proposals for large equipment and facilities. Please omit funding for “indirect costs” in your budget and requests.)
    • Amount of dollar grant requested
    • An itemized budget sheet of the expenses that total to the requested grant amount
    • The sponsoring organization’s commitment (contributions in-kind during and after the period of the grant)
    • Other organization you applied for project funding including the requested amount
  5. Attachments
    • The sponsoring organization’s brief history
    • Letter of tax exemption under the IRS 501(C)(3)
    • Current independent audit
    • The sponsoring organization’s current annual operating budget
    • Current IRS for 990
    • Most Recent Board of Directors, including their professional affiliations or business and frequency of their board meetings
    • Current Annual Report
    • List of other foundation supporters or major business in the last 3 years
    • Resume of Director or Project Manager

The application and its attachments (original and one copy) should be sent by mail or delivered to the following address. The foundation must receive the document not later than July 15, 2011.
